What Does It Mean to Be Transgender?

When you think of yourself as male or female, it's called gender identity. Everyone has a gender identity — the inborn sense of ourselves as being male or female.

Most people's gender identity matches their anatomy. But people who are transgender feel different from their physical appearances.

What society expects of us as men, women, boys, and girls also affects what we feel about ourselves. Every culture has "rules" about what is expected for men and what is expected for women. These expectations can include things like hairstyles, clothing, and jobs — and how people should act or behave.

Society doesn't have to state these "rules." Everyone knows them because we see them all over. So most of us grow up believing men should act a certain way and women should act a certain way without thinking about it much. Transgender people, though, have a very different sense of themselves.

Some transgender people know they feel "different" from the time they're young kids. Others start sensing it around puberty or even later. When people who are transgender become aware that they feel mismatched with their bodies, they may feel confused and emotionally conflicted.

Some people decide to physically change their bodies — through surgery or taking hormones — to match the gender they feel they really are. Physically becoming the opposite gender can be a long, complicated, and expensive process.

Not everyone decides to get surgery or hormones, though. Some transgender people are most comfortable keeping their physical anatomy but dressing as the opposite gender. Some aren't completely sure what they want yet, but may start by asking to be called a new name and use the pronouns that go with that name (such as "Amanda" instead of "Anthony" and "she" instead of "he").

Once transgender people start living their lives as the opposite gender, many issues may come up — like how to fill out forms that require checking "female" or "male," and even which public bathrooms to use.

As with any group, not all transgender people want or think the same things. It all depends on what that particular person needs to feel most comfortable in both body and mind.

How PMS Works

Every month like clockwork, some women come down with a strange collection of symptoms. They may become irritable, snapping at their family and friends without the slightest provocation. They may forget where they put their car keys and have trouble paying attention during meetings at work. They may crave chocolate or find themselves sneaking a few pieces of pizza after their children have gone to bed. Or they may suddenly hate the way they look because they feel bloated and their hair looks oily and lifeless.

We are quick to give these symptoms a label: premenstrual syndrome, or PMS. But what is PMS? Some people dismiss it as "that time of the month" -- an excuse that women give in order to eat tons of chocolate and get away with acting a bit cranky.

But doctors recognize PMS as a real medical condition, with real physical and psychological symptoms. For many women, the symptoms are debilitating enough to interfere with their lives.

In this article, we'll discover how doctors identify PMS, learn the underlying causes of this ailment and find out how women who suffer from it can find relief.

What is PMS?

Premenstrual Syndrome (PMS) is a set of hormonal changes that trigger a wide range of physical and emotional symptoms in women. These symptoms can range from anger and irritability, to abdominal cramping and breast tenderness, to food cravings. PMS generally occurs between 7 to 14 days before menstruation (a woman's monthly bleeding, or "period") and then stops once menstruation begins. It can only occur during a woman's childbearing years -- after menopause, it stops.

Up to 40 percent of menstruating women experience symptoms of PMS [ref]. In most of these women, PMS is nothing more than an annoyance. But in about 5 percent of women, the symptoms are debilitating enough to interfere with daily life.

There are more than 150 different symptoms associated with PMS, so it is often difficult to diagnose. Although there is no real test for PMS, doctors will do tests to rule out other conditions that cause similar symptoms. When a woman's symptoms coincide with her menstrual cycle, and she tests negative for other conditions, PMS is usually determined to be the cause of her symptoms. Keeping a monthly diary of symptoms is helpful to confirm the diagnosis.

A new documentary about Bali's legendary beach, Cowboys in Paradise, led to a crackdown on the trade and the arrests of 28 "tanned and muscular men" this week. Lonely, sunburned European women with vacation braids are totally devastated.

Bali's "Kuta Cowboys" are widely known among the Lonely Planet-toting backpacker set for being incredibly and way more attentive than boring, pasty chumps back in Birmingham or Dusseldorf. They might strum a romantic tune on their guitar at sunset, or give you a ride across the island on the back of their scooter, or maybe even introduce you to their wife and kids before heading into their room for some time. But after a trailer for a documentary on the Kuta Cowboys hit YouTube earlier this week, police decided enough was enough and started arresting the smooth talking loverboys.

The director of Cowboys in Paradise, Amit Virmani, told the AP: "A witch hunt for men with tanned and muscular bodies on the beach is the last thing anybody wants." Just ask the suddenly single, bored youth hostel guests across the island.

Trial Shows First-Ever Reduction in HIV Infections with Microbicide Candidate; Other Studies Indicate Effectiveness of PrEP against SHIV Infection in Animals

A new study released at the 16th Conference on Retroviruses and Opportunistic Infections (CROI) in Montreal, Canada, shows that a microbicide candidate or gel known as PRO2000, designed to lower the risk of HIV infection when applied before intercourse, appears to have a 30 percent level of effectiveness in preventing HIV infection. These findings are promising, although not statistically significant, and additional evidence is needed to conclusively determine whether PRO2000 is an effective microbicide.

Data from the multi-site clinical trial, known as HPTN 035 and conducted by the Microbicide Trials Network, showed that women who received the PRO2000 candidate, plus condoms, had 30 percent fewer HIV infections than those who received condoms or condoms plus a placebo gel. According to Dr. Salim Abdool Karim of the Center for the AIDS Program of Research in South Africa, “It’s very exciting that PRO 2000 might have a positive effect. Now we need a trial that’s big enough to show whether this is a statistically significant effect.” Researchers conducting a separate efficacy trial of PRO2000 in South Africa, Tanzania, Uganda and Zambia, which is in its final stages, will soon obtain more definitive data regarding the candidate’s effectiveness. Data on another microbicide candidate tested in the HPTN 035 trial, BufferGel, did not show a reduction in HIV infection risk among women.

In other HIV prevention news, researchers at the U.S. Centers for Disease Control and Prevention (CDC) presented study findings at CROI showing that two different strategies of pre-exposure prophylaxis, or PrEP, were effective in monkey studies. One study showed for the first time that the oral administration of Truvada, a combination of two antiretroviral drugs (ARVs), before and after exposure to SHIV, a hybrid of HIV and its monkey equivalent SIV, effectively prevented rectal infection in most rhesus macaques. A second study found that a single-drug gel was just as effective as a two-drug gel in preventing SHIV infection in female macaques. SHIV infection is generally considered an easier target than SIV infection, at least for vaccines.

IAVI strongly supports the continued research and development of both microbicides and PrEP as potential prevention tools to combat HIV and AIDS. An effective microbicide could increase the ability of women to protect themselves from HIV. Evidence to date provides a strong rationale for exploring PrEP as a new tool for reducing the risk of HIV infection. With 7,500 new HIV infections a day, the development of new HIV prevention strategies, including an AIDS vaccine, remains urgent.

After speaking awkwardly for 10 minutes about the differences between boys and girls, Jimmy's mom asked if he wanted to know anything else. "Yes," he said. "Kim said she was Chinese. What does that mean?" Although Mom told Jimmy more than he wanted to know, experts agree that he'll be OK because he has a parent (and maybe two) willing to have a frank discussion about. That's the key to shaping a healthy attitude, they say.

Teaching From the Time They're Tots
According to Schwartz, the time to introduce the subject of is when a child wants to know what their "peepee" is. By talking to your children in a developmentally appropriate way, you remove the taint of taboo, she says.

Experts recommend that you consider buying a children's book on to guide you through the tougher topics, and when possible broach a related subject in terms of a TV show or movie you and your child have seen, or a book he or she has read.
